New York State requires that all campers have a complete immunization record on file. All campers who have not previously registered at Camp Hunt are required to do so. Medical forms are available below to be completed before or during registration.

In addition, no camper may be admitted into the program until permission for emergency medical care has been obtained. All medical expenses will be referred to the parent or guardian for payment. Camp Hunt is not financially responsible for the medical expenses incurred due to accident or illness of any camper.

Note: No medications (prescriptions or over the counter) can be administered at Camp Hunt without a written doctor’s order.
